Seaver College

At the beginning of the semester, students should give a copy of a current accommodation letter to all professors in whose classes they wish to receive exam/quiz accommodations. Professors may be able to provide accommodations. If not, please:

  1. Complete an Exam Proctoring Form (EPF) using these instructions
  2. Two weeks prior to the exam, the OSA will contact the professor to get the exam details
  3. Check email and google calendar appointment one week prior to exam date for a confirmation (the title of the event will say "Confirmed")

Please note:

  • It is the responsibility of the student to confirm that the exam has been scheduled with the OSA
  • Re-scheduling of proctored exams is allowed only with written permission of professor (late arrival may result in a shortened exam time)
  • Personal belongings are not allowed in the testing room; OSA is not responsible for items left in the OSA lobby
  • The OSA must receive your Exam Proctoring Form at least TWO WEEKS ahead of the exam date or the office will not be able to guarantee accommodations

Please note:

  • If you have not heard from the OSA five days before the exam, please contact the Administrative Services Coordinator
