F-1 Practical Training


Name of Form Purpose of Form Who Should Use Form Requirements
CPT Request Submit this form to the OISS before participating in an internship, externship, practicum or other work required for a course, known as "curricular practical training (CPT)." Starting CPT without permission is a violation of F-1 status. Graduate students who are enrolled in a course requiring CPT work. Undergraduate students should go here for the CPT process Letter from company, school or organization where CPT will be performed; name and e-mail address of your academic advisor or practicum coordinator, who will provide certification that your CPT is required.
OPT Request Submit this form to the OISS about 4 months prior to completing your program of study to request a new Form I-20 recommending employment authorization in your major field of study, known as "optional practical training (OPT)." Students who plan to apply for OPT. Please refer to the OPT Manual about 4 months prior to expected program completion date. Students should read the OPT Manual  carefully for information about OPT, instructions on how to apply, and regulations on maintaining status while on OPT.
STEM Extension Request Submit this form to to the OISS to request a new Form I-20 recommending an extension of your OPT based on a science, technology, engineering or mathematics (STEM) major. Students currently on OPT, who have graduated with a STEM eligible degree, who want to apply for the STEM Extension. Please refer to the OPT Manual for information on how and when to apply. OPT Information Update Submit this form to report the receipt of your EAD card, new employer, changes to information of current employer, or a new employer Students who have applied for OPT or are currently on OPT A scanned copy of your EAD card

eShipGlobal Submit this form if you've selected to have a new I-20 sent to you via eShipGlobal  Students who have requested a new I-20 sent to them via eShipGlobal It is necessary that you go to eShipGlobal to prepay for the shipping label here 
Exit or Transfer Request Submit this form if you are requesting a school transfer or completing your F-1 status All students who are either transferring or completing F-1 Status  Make sure you have updated all of your information by completing the proper forms (address update, OPT report, etc.)
Program Extension Request Submit this form if you need to extend your program end date. This must be done before the program end date on your most recent I-20 has passed  Students who need to extend their program end date  You will need approval from your program/academic advisor
Reduced Course Load for Academic Reason Submit this form if you cannot maintain full time student status due to academic reasons Students who: have initial difficulty with English language or reading requirements, unfamiliarity with U.S. teaching methods or improper course level placement You will need approval from your program/academic advisor
Reduced Course Load for Last Term  Submit this form if you are in your last term and the required units to graduate do not meet the full time student requirement  Students who are in the last term and are not taking units to maintain full time student status  You will need approval from your program/academic advisor
Reduced Course Load for Medical Reason  Submit this form if you cannot maintain full time student status due to medical reasons  Students with a valid medical condition, that will require them to fall below full time status  A medical note from your doctor 
Travel Permission or I-20 Update  Submit this form when you travel out of the country, to ensure you have the proper permissions to re-enter the U.S. You can also submit this form to replace a lost I-20, or to receive a copy of an I-20 that has updated information  Students who: are traveling out of the country, have lost their I-20 and want to request a new one, have updated information on their I-20 and want to request an updated one  
International Student Data (ISD)  Submit this form to request your initial or transfer I-20 as an incoming student to Pepperdine University  Students who are attending Pepperdine University for the first time, and are requesting a new I-20  Passport and financial evidence (including proof of scholarship if applicable)
